New Patient Appointment
Please arrive 15 minutes prior to your scheduled appointment to complete the new patient paperwork. Photo ID and insurance card is required.
Patient Balance
Co-payments, Coinsurance, Deductible and any other patient responsibility indicated by your insurance carrier is due at the time of service. You are responsible for knowing your insurance plan. If you are not familiar with your plan coverage we recommend you contact your carrier or plan provider directly.
Insurance
It is important to notify us as soon as possible of any changes related to your insurance coverage. Failing to do so may result in unpaid claims, and you will be responsible for the balance of the claim. Orthopaedic Spine & Joint Center does not accept responsibility for incorrect information given by you or your insurance carrier or other plan provider regarding your insurance benefits or benefit plans.
Missed Appointments Missed Office Visit appointments and cancellations without a full 24-HR notice, will result in a $65.00 fee.
Missed Procedures, Injection appointments, and Cancellations without a full 48-HR notice, will result in a $90.00 fee.

Patient Portal gives patients 24/7 access to patient medical records, labs, and secure messages, anywhere and on any device. Patients can update demographics, complete forms and questionnaires, request prescription refills, access educational materials, schedule appointment, and make a payment on your account.
